Mail-In Entry Form

Statement of Release:  Quiltmakers submitting works grant Sacred Threads 2009 and local media permission to photograph the works for advertising, promotions and own personal use any works that are on display. Sacred Threads may also select your piece for magazine articles, images on the website, an exhibit booklet, and a CD for purchase.

I have read and agree to give Sacred Threads permission to publish pictures of my entries for publicity and publication.

For each quilt please submit the following information on a separate 8.5 x 11" piece of paper. Also include the information in a file on your CD.

• Name and address of the Artist
• Name of the Quilt
• Dimensions of the quilt (width x length in inches)
• Category for the quilt
• Artist’s statement of 100 words or less
• One sentence summary of the artist’s statement for inclusion in the show booklet.

Please also include a written appraisal if the quilt value is over $1000.

Please be sure and choose a category (joy, spirituality, inspiration, peace/brotherhood, healing, or grief) for each entry. Your artist’s statement is an important part of the jurying process and a separate statement for each quilt must be submitted with your entry form. Please type your statement or print legibly.
